Run a blameless incident postmortem

The postmortem after an incident is where a team decides whether it happens again. The Incident review template gives it the usual shape — what happened, when, why, and what changes — so the time goes on the conversation, not on the document.

The Incident review template is on every plan, the free one included.

The Incident review template: Summary, Impact and Detection; a Timeline; then Contributing factors, What went well and Actions.

How to run it

  1. 1In Kaavit, choose New board and pick it under Start from, or use Insert template on the toolbar of a board you already have.
  2. 2Before the review, one person drafts the timeline: one sticky per event, left to right, each with its time. Or paste the log into the AI prompt and ask for the timeline.
  3. 3In the review, fill Summary, Impact and Detection together, and correct the timeline where people remember it differently.
  4. 4For Contributing factors, ask why of each answer until it is about the system rather than a person. Select the timeline and ask the AI which factors it points to, as a first draft to argue with.
  5. 5End with Actions: an owner and a date for each, in the checklist.

Share what you learned

Send the rest of the company a view-only link, or one that lets them comment, and revoke it whenever you like. People can pin a question to a sticky without interrupting the review.

Questions

Is this a root cause analysis?

It does the same job. The template says contributing factors rather than root cause, because an incident rarely has only one — but ask why of each factor until the answer is about the system, and the postmortem has done what a root cause analysis does.

Does Kaavit connect to our alerting or incident tool?

No. Add the events by hand, or paste an export into the AI prompt and ask for a timeline.

Can we keep past reviews together?

Yes. Each review is a board in your organisation's board list, with its comments and version history. Tag them to find them later, or link them from a project hub.
